We provide alternative pathways for at-risk youth, restoring a positive connection with the community, thereby creating a safer, healthier Chatham-Kent. Clients must be between the ages of twelve (12) and seventeen (17) to participate in the Pre-Charge Diversion Program offered through Restorative Justice Chatham-Kent. This ‘alternative measures’ program is funded by the Ministry of Children ﹠ Youth Services and is delivered in conjunction with the Chatham Kent Police Service, as they diligently screen and refer the appropriate youth cases to our agency. “Positive Strides“ Intervention ﹠ Educational Programming All program modules are available to those youth participating in our Pre-Charge Diversion Program (12–17). Intervention programs are also available to those youth (10–17) in the community who are not currently in conflict with the law, but who are at risk for delinquency or criminal behaviour; youth who are at risk to recidivate + become in conflict with the law again, are also eligible to participate in offered programs. Programs can be tailored to any client’s particular learning style, and are delivered in INDIVIDUAL, one-on-one format with our Program Coordinator. NOTE: Programming sessions run for 45 minutes – 1 hour in length, typically on a weekly basis; however, they are scheduled around the client’s needs. A youth can attend programming for as many sessions as they feel are beneficial, but the average is approximately 5 sessions, in addition to an initial intake assessment with a parent/guardian. Programming services can be delivered in alternative settings if needed; for example, in the client’s place of residence (outside of Chatham-proper) at a Municipal Service Centre, or at the client’s school, in the case of transportation issues. In April 1971, Albion Neighbourhood Services (formally called Rexdale Information Directory) office opened in a donated space at The Albion Centre/Mall, with trained volunteers answering inquiries from the public. Later Ernestine Van Marle was hired as the Co-coordinator. The first secure funding came about in 1976, after the volunteer Board of Directors successfully applied for United Way Toronto agency membership. Becoming more than an information directory… From its initial humble beginning, callers’ inquiries indicated a need for a legal service. This prompted the agency to develop its first partnership, and as a result Ontario Legal Aid began providing services from the office in The Albion Centre/Mall, (this service grew to become an independent organization in 1993, the Rexdale Community Legal Clinic). Over the years, a number of other services were developed and implemented in response to the changing needs of the community. Services such as crisis intervention, pension advocacy, settlement assistance, income tax form filling, housing help, children and youth programs and summer day camp. In 1994, the agency began to develop and apply for membership for a Boys ﹠ Girls Club, with the assistance of the Ontario Trillium Foundation and Boys and Girls Clubs of Canada. Finding a new name… To reflect the growth and the diversity of the services being offered, as well as the initial and still existing location in the Albion Centre/Mall, the name of the agency was legally changed in 1998 to Albion Neighbourhood Services. Albion Neighbourhood Services has been in existence since 1971, and continues to serve the South, North Etobicoke/Jamestown/Rexdale community and other areas of the City of Toronto in assisting residents to lead healthy lives and in building individual and community wide capacity. Today, Albion Neighbourhood Services has 4 major departments delivering programs and services that serve a diverse range of multi-cultural individuals and families: Access, Housing, Boys ﹠ Girls Club and Trusteeships.

Toronto Central Community Care Access Centre (CCAC) delivers home and community health care and connects people to other services in our community. We work with seniors, adults, children and their families to help determine and provide the right care and health supports to keep them at home, where they are happiest, for as long as possible. Toronto Central CCAC’s services are available to over 1.5 million residents in the Toronto Central LHIN geography, resourced by our three regional offices. Approximately 46,000 Torontonians of all ages and cultural backgrounds benefit from home care services delivered by Toronto Central CCAC every year. We serve an area with the highest rates of low income and single parent families, as well as a diverse urban population. We work with 26 hospitals, 150 community-based case coordinators, 42 Long-Term Care Homes and 13 Community Health Centres. We also work with 35 health service provider organizations to deliver a range of in-home and in-school services to our clients (e.g. nursing, occupational therapy, physiotherapy, speech language pathology). In any given month Toronto Central CCAC supports: · More than 19,000 people of all ages, cultures and backgrounds · 1,700 children getting support at their schools · 400 adults receiving rehabilitation services · 23,000 information and referral inquiries · The transition to a long-term care home for 240 clients · 600 individuals to die at home with dignity · Saving 1,000s of hospital days by transitioning 7,000 clients home for care. Over 500 people work for the Toronto Central CCAC at three community offices and at 26 hospital sites in order to better serve our clients. La mission de la Fondation est d’apporter « le petit plus qui fait la différence » aux jeunes de l’Estrie qui doivent reconstruire leurs espérances. Ces enfants et ces adolescents ont en commun un passé difficile et se battent pour rebâtir leurs rêves. Beaucoup d’entre eux ont été victimes d’abus, de violences physiques et psychologiques, de négligence et d’abandon. Le Centre jeunesse de l’Estrie (CJE) a dû les prendre sous son aile. Ces enfants demeurent souvent marqués par ce début de vie difficile : retards de développement, anxiété, sentiment dépressif, méfiance envers les adultes, difficulté d’attachement, retard de langage, difficulté au niveau de la de socialisation ou isolement. Ils doivent réapprendre l’enthousiasme et l’envie de construire demain. La Fondation est là pour les appuyer dans leurs projets. En parallèle aux actions d’accompagnement menées par le CJE, la Fondation supporte financièrement ces « petits plus » qui permettent à près de 175 jeunes par année de reprendre goût à la vie : - vivre des expériences enrichissantes dans le domaine des sports et des arts, - poursuivre leurs études, - être mieux outillés pour débuter leur vie autonome lorsqu’ils atteignent la majorité. Diagnosis and treatment are improving, allowing people to live increasingly active and productive lives.
